Trainee Brain Injury Case Manager

Starting Salary £26k PA plus Performance Related Pay Scheme

Are you an Occupational Therapist looking to move into a rewarding role as a Trainee Case Manager on a private basis?

We have an excellent opportunity for a Trainee Case Manager to work alongside our experienced team, with a view to progressing to the role of Case Manager.

Our clients are adults or children who have sustained brain injury, spinal injury or amputation or have other complex injuries or conditions. We will provide training and mentoring for the right candidate to enable development and progression.

The role will be based mainly at our office in Clapham, south London, during office hours, with travel around the south-east to clients in the community.

As a Trainee Case Manager you will initially be working alongside experienced Case Managers in the following:

• Performing Immediate Needs Assessments with injured clients at their home or in hospital.
• Detailed report writing.
• Desk-based research and coordination.
• Sourcing and coordinating a mixture of statutory and private services to meet the injured client’s needs.
• Maintaining detailed and accurate documentation.
• Assembling and overseeing packages of care and support, from low level support to 24/7 care teams.
• Supporting with the management of rehabilitation programmes, sourcing professionals and making referrals.
