Job description

We are looking for a very experienced nurse with recent acute background to be able to engage clinically with our hospitals, and be able to demonstrate high levels level of confidence and initiative to be a case finder.

Role Objective   

To work within the Recovery at Home services provided by the Company as part of an ongoing partnership with the NHS. Dual responsibility and internal rotation working within a multi disciplinary team, with continuing responsibility for a defined caseload, including the assessment, development, implementation and evaluation of care plans for adults with medical and social care needs and who require care on the home pathway. To negotiate for and coordinate resources on behalf of the patient who is deemed eligible for the home pathway.

Responsibilities include:

  • Participate in setting and maintaining standards of professional competence and current nursing practices.
  • To manage a defined caseload of patients with medical and social care needs.
  • Work within multi-professional teams across all agencies involved in the patients care to establish their involvement and patients assessed needs.
  • To produce written, electronic comprehensive nursing assessments in a timely manner.
  • Deliver efficient and effective care and communication between the multidisciplinary team, the patient and carers.
  • To present comprehensive nursing documents through high level clinical decision making.
  • To make recommendations regarding eligibility for homecare pathway service.
  • Work in partnership with patients and carers to enable them to make informed choices about care and treatment to promote their health and well being.
  • To act as an independent nurse assessor and patient advocate
  • Work as a member of the team providing a high standard of care to patients and carers.
  • Maintain continuity of care from the acute setting to the home setting
  • To ensure a valid and up to date NMC Registration, HCPC Registration or any other required Professional Registration is held and maintained at all times.

Knowledge and Experience:

Significant experience in:

  • Evidence of post registration experience of working in a busy environment
  • Evidence of continued professional development
  • Evidence of high level clinical assessment skills and decision making
  • Evidence of high level clinical skills eg complex wound management, cannulation and venepuncture.

The post holder should have knowledge of clinical governance and the application of risk management.

Qualifications:

The following qualifications are essential:

  • RGN part 1 NMC Register
  • Experience in an acute or community setting (relevant ENB post registration)

The following qualification is desirable:

  • Post graduate qualification eg Facilitated Learning and Assessing in Practice or specialist practice certificate.
  • Evidence of continuing professional development
  • Cannulation and phlebotomy skills desirable
  • Experience in wound care, orthopaedics, respiratory therapies desirable.
