Job Title: Healthcare Navigator

Location: East London Borough

Contract Details: Temporary, Full Time

Salary: £17.54 - £21.72 per hour

About Our Client:

Join our client, a London Borough Council, and play a crucial role in the resettlement of rough sleepers by providing specialist intervention and a multi-disciplinary approach. As a Healthcare Navigator, you will take a leading role in coordinating interventions and working closely with the existing Street Outreach Team and other partners.

Responsibilities:

·  Offering early intervention and ongoing support to prevent individuals with multiple and complex needs from returning to rough sleeping after hospital discharge.

·  Supporting individuals with complex needs, such as rough sleeping, substance misuse, and physical and mental health needs

·  Collaborating with caregivers, CPNs, and other facilities and services to address mental health concerns

·  Coordinating access to general practitioner, dentist, specialist healthcare, substance misuse and detox programmes, and mental health services

·  Providing advice and support related to welfare, debt, and budgeting

·  Offering guidance and assistance in accessing training, education, employment opportunities, life skills, and independent living training

·  Helping individuals rebuild social networks with family and friends

Essential:

·  Experience in providing support to vulnerable individuals with complex needs, preferably within a housing or homelessness setting.

·  Strong knowledge and understanding of housing and welfare rights.

·  Excellent communication, interpersonal, and problem-solving skills.

·  Ability to work collaboratively as part of a team and independently.

·  A compassionate and non-judgmental approach to working with individuals facing challenging circumstances.

Desirable:

·  Experience dealing with safeguarding and health and well-being issues.

·  Knowledge of local support services and agencies.

·  Understanding of the causes and impacts of homelessness.

·  Knowledge of relevant legislation and policies related to housing and homelessness.
