Enablement Officer - LOCAL AUTHORITY
Wandsworth, England
Monday- Friday 9.00am-5.00pm
£11.83 hourly pay rate
ASAP start!
3 month temporary assignment

Main duties shall include but are not limited to:

*To be responsible for the day to day management and supervision of designated areas of the KITE service, undertaking regular supervision with a team of carers providing enablement, ensuring that they receive regular quality checks of their work and annual appraisals.

*To be responsible for preparing and planning rotas effectively, ensuring continuity and quality of care at all times and rotas planned in advance of at least one week.

*To be part of an out-of-hours rota when required.

*To carry out first visits to adults receiving the service and complete Health & Safety Risk assessments, ensuring that they are supplied with an Enablement fact sheet and required documentation. Discuss the Service Users Enablement Plan with them before obtaining their consent to it, and deliver care record sheets, medication charts and other relevant paperwork.

*To work in partnership with social care, health and any other professionals to contribute to the goal setting, review and transition of people through the service.

*To maintain accurate case records/ case notes and ensure that Enablement plans are up-dated as people make progress towards achieving their goals whilst maintaining excellent communication skills with Service Users, colleagues and partnerships.

*To be responsible for ensuring that records are collected from the person's home at the end of the service and that termination forms are completed and that audit of record books is completed.

*To have a good understanding of CQC Fundamental standards, health and safety, medication and manual handling procedures.
