We're looking for an enthusiastic and experienced Social worker for our client in Sutton to provide assessments and support as part of their Multi-Disciplinary team to facilitate discharge.

Sutton's Hospital Pathway team in Adult social care supports Sutton Residents through the whole process surrounding hospital admission, enabling individuals to regain independence.

Duties and Responsibilities:

·  Social workers are required to complete short assessments in crisis to ensure appropriate support is provided in a timely manner.

·  Working closely with the in house reablement team to provide holistic and strength based eligibility assessments to conjunction with the multi-disciplinary team.

·  Social workers will also be required to complete Mental Capacity Assessments and Best Interest Decisions.

·  Contribute to NHS continuing Health care assessments.

·  Supporting individuals to address issues relating to the habitability to their property, and make applications to Court of Protection regarding deputyship for finances.

To be a successful applicant within this position you would need to have at least 2 years post qualifying experience within Adults social care as a Qualified Social worker, and be registered with the HCPC.
